Canada Life - 60 Min. Initial Assessment

Manitoba Blue Cross - 3rd party billing. Service fee includes taxes.

* member / spouse / dependent

Blue Cross - 60 Min. Follow Up Session

12 athletic therapy treatment sessions (60 minutes each) for 845.00 (save 175.00)

8 athletic therapy sessions (90 minutes each).

Please advise if treatment sessions being shared with family members.

Follow-up treatment.

8 x 90 minute AT Treatment Session Package Deal

5 athletic therapy treatment sessions for 390.00 (save 35.00). Follow-up treatment.

Manitoba Public Insurance - Injury Claims (MPIC) - 3rd party billing. Patient is not responsible for service fee upon confirmed coverage of injury claim with MPI.

We require the following information prior to first appointment;

* First and last name
* Injury Claim number
* Date of loss (mm/dd/yyyy)
* Birthdate (mm/dd/yyyy)
* MPI case manager name and phone number (if applicable)
